A care plan app for family practice teams

Patients record home readings beside the plan a physician signed. Readings outside a signed target wait on one list for your team’s next review.

The care team’s screen for a fictional patient on Sep 18. For team review: movement below the signed target from Sep 12 to 18, and systolic blood pressure above the signed target from Sep 5 to 18, both new and not assigned. Under it, the plan over the last 14 days shows each part in the plan’s words with the patient’s readiness, starting with movement, ready, 0 of 3 entered days in range, and nutrition, getting ready, eating manageable on Sep 17.

The plan is signed in the clinic and followed at home

The readings in between often reach you as a memory at the next visit. With Loopstead, patients record them beside the plan you signed, and your team reviews them before the patient comes back.

What your team sees

Patients see the plan in the words a physician signed.

How it works

  1. At the visit, sign the plan

Start from the visit note you already wrote. Paste it, and its sentences are placed under Movement, Nutrition, Sleep, Water or Breathe by heading and keyword. You check each part, then sign.

The care plan for a fictional patient, as the physician sees it, with a button that reads Update plan from a note. The Movement row reads build a steady walking routine: go for a comfortable 15 minute walk after lunch each day, and stay within the limits you talked about with your doctor. Its target is 15 to 30 minutes a day, and it is signed.

  2. At home, patients record readings

Patients type or say a reading, beside the goal a physician signed.

The blood pressure tile in a fictional patient’s app: 140/88 mmHg on Sep 17, typed in by the patient. Your goal: top number 90 to 135 mmHg. Home readings in the last 30 days: 2 in range and 5 above, compared with the range the doctor set.

  3. Before the next visit, open the brief

It shows home readings and each part of the plan.

Home blood pressure in the visit brief for a fictional patient on Sep 18: 2 of 7 readings in target over the last 30 days, average 137/86 against a target of 90 to 135 mmHg. The chart shades the systolic target; the solid line is systolic and the dashed line diastolic.

Questions practices ask

Where is patient data kept?

Loopstead is designed to keep patient data in Canada. See Privacy & data.

Does it connect to our EMR?

No. It runs in a web browser beside your EMR.

What happens if a reading is high?

It waits for your team’s next review: before each visit, weekly or monthly, as the physician chooses. The app tells patients to call their clinic or 911 in an emergency.

Can patients message us?

No. Notes they add about their readings wait beside them for your review.

Who can change the plan?

A physician signs it. Practice nurses help prepare it but can’t sign, approve or publish. Patients see a plan or target only after a physician approves it.

What do patients see?

The plan in the words a physician signed. “Read my plan aloud” can read it to them, and they can skip any question when they check in. See a page for your patients.

Can family members see readings?

No. If the patient shares, a family member sees a weekly summary and the words of any goals the patient picks.
